Brian, I agree. I noticed some visual glitches as well but I couldn't think of how to mention them without giving away too much.

SPOILER FOLLOWS:

The biggest mistake I noticed is that Ethan's finger magically grows back after he is arrested and during pretty much any of the game's endings.

I agree with a lot of what you said, this is a fantastic game that draws you in and makes you experience everything the characters are going through. I would suggest playing this game with little to no external lighting other than the t.v. for a more immersing experience.
The game play is a fresh direction from the usual button mashing or repetitive actions that are the norm but it is hardly "ground breaking" as this same method was used back in the days of Dragon's Lair and the like. We just haven't seen it in so long that it has once again become fresh and new.
The graphics were hit and miss with me. Most of the settings and motion capture was pure genius but then a glitch would pop up and remove the player from the experience. A child at a table eating dinner and scooping up food even though his spoon is 4 inches above the plate, or running finders through your hair and seeing through the hand/hair, and many other things just not matching up contradict the years of care that obviously went into this game. It leaves one with a feeling of shortcuts being taken or lacking quality control.
However this is not enough to fully detract from such a cool game.
